By completing this commitment form and paying the $25 commitment fee, you agree to specified terms through the Create Birmingham OMBE Business Certification Program.

PROGRAM PARTICIPANT TERMS:

1. Fill out the contract below.
2. Pay a $25 commitment fee by Wednesday, April 30
3. Attend all 6 sessions.
   • Session 1: Thursday, May 8, from 4:00 – 5:00 pm
   • Session 2: Thursday, May 15, from 4:00 – 5:00 pm
   • Session 3: Thursday, May 22, from 4:00 – 5:00 pm
   • Session 4: Thursday, May 29, from 4:00 – 5:00 pm
   • Session 5: Thursday, June 5, from 4:00 – 5:00 pm
   • Session 6: Thursday, June 12, from 4:00 – 5:00 pm
     Business Owner must be in attendance to sign with a Public Notary

Please note: Any absences will forfeit the return of your $25 commitment fee.
