By completing this commitment form and paying the $25 commitment fee, you agree to specified terms in order to fulfill services through the Create Birmingham, Create Consults Program.

PROGRAM PARTICIPANT TERMS:

1. Pay a non-refundable $25 commitment fee by March 31.
2. Complete the required Create Foundations pre-work by April 12.
   • Failure to complete pre-work by the specified date will result in forfeiture of participation in the Create Consults program.
3. Complete the required Create Birmingham intake session.
   • If a reschedule is necessary, a 24-hour notice, in writing, is required.
4. Attend all scheduled consulting sessions by June 15.
   • Tardiness will not be tolerated and is counted as an absence.
   • If a reschedule is necessary, a 24-hour notice, in writing, is required.
5. Complete exit interview with Create Birmingham staff member.

Please note: Any absences (for intakes and/or consulting sessions), without 24-hour notification will result in immediate withdrawal from the Create Consults program. Your resources will be reallocated to another applicant on the waiting list, and the $25 commitment will not be refunded.

CREATE BIRMINGHAM TERMS:

1. Provide access to pre-work at least 2 weeks prior to the deadline.
2. Schedule and complete 1:1 intake session.
   • If a reschedule is necessary, a 24-hour notice, in writing, will be provided.
3. Identify and connect you with appropriate consultants via email.
   • Each participant will have access to a maximum of $3,000 worth of 1:1 consulting hours. This totals ten hours and will be determined by Create Birmingham.
4. Complete the exit interview with the participant.
